Scrotal Tightening (Scrotoplasty): Surgical Tightening of the Scrotum

Tightening of a slackened scrotum is an operation that many men do not have in mind when they first look into intimate surgery. Nevertheless, it represents an exciting and safe technique for genital rejuvenation — and, in combination with the penile webbing correction, it brings a surprising side effect: The penis appears optically longer – the scrotum fuller and younger! At the Praxisklinik am Rosengarten, a proprietary technique was developed in which the scar comes to lie exactly in the midline of the scrotum — thereby becoming practically invisible. The result: a firmer, more youthful scrotum, a corrected penile web, and full optical length of the penis.

Why does the scrotum slacken — and when is surgery advisable?

The scrotum is a skin pouch outside the body that contains the roughly plum-sized testicles. Interwoven with muscle fibers, it actively regulates the temperature of the testicles — it contracts in the cold and relaxes in warmth to keep sperm production at the optimal 34–35°C.

With increasing age, the skin loses elasticity and firmness — the scrotum descends. Genetic predisposition, significant weight loss, or hormonal changes can also lead to an excess of scrotal skin. The result: the scrotum hangs low, appears slack, and can lead to concrete complaints in everyday life.

The penile web: Why scrotal tightening optically lengthens the penis

Many men do not know that a flap of skin can run between the penile shaft and the scrotum — the so-called penile web. In the English-speaking world, this structure is called "penile webbing". This skin, which disturbs the testicle-penis angle, can cause the penis to look considerably shorter than it actually is.

As part of scrotal tightening, the penile web can be precisely corrected — the skin between the scrotum and penile shaft becomes taut and the penis comes into its full length. This optically penis-enlarging effect is a convincing additional benefit of the operation.

Important note for men planning a penis lengthening

If you are planning a surgical penis lengthening (ligamentotomy), please check whether you have a penile web.

Dr. Spanholtz recommends definitely correcting the penile web at the same time — only then does the maximum result of the penis lengthening come fully into effect.

Both procedures can be easily combined in one session.

The technique of the AndroKlinik and Dr. Spanholtz: The scar in the midline

What distinguishes scrotal tightening at the Praxisklinik am Rosengarten from many other providers is the specially developed suture technique: The scar is positioned so that it comes to lie exactly in the natural midline of the scrotum — the line that naturally divides every scrotum.

This midline is a natural anatomical fold — a scar in this location is practically undetectable after complete healing. During the informational consultation, patients also receive the necessary care products so that the scar heals optimally and invisibly.

The 3-dimensional scrotal tightening — a further development by Dr. Spanholtz

The classic scrotal tightening corrects the hanging, overly long scrotum — and thereby already achieves a significantly more youthful overall appearance. A further development created by Dr. Spanholtz goes one step further: the 3-dimensional scrotal tightening.

It addresses excess skin that many men carry in a place that is often overlooked: lateral to the penile shaft. These lateral folds hang in longitudinal folds beside the penis — like a lion's mane — and give the genital area a slack-looking, not very youthful appearance. Even if the scrotum itself is tightened, this appearance remains incomplete without correction of the lateral folds.

What exactly are these lateral folds?

The skin that descends laterally from the penile shaft in longitudinal folds is anatomically part of the transition area between the penis and scrotum. With increasing age, after weight loss, or due to genetic predisposition, this skin can become excessive and fall downwards — visible as lateral folds that make the penis appear shorter and the entire area less defined.

Many men who wish for a penis lengthening or penis thickening experience a good result after the operation — and only then notice that the lateral folds still detract from the overall appearance. The 3D scrotal tightening closes this gap.

The operation — gentle, precise, without visible scars
The technique developed by Dr. Spanholtz removes the lateral excess skin in a way in which the scars disappear into the natural transition area and are practically no longer detectable after healing. The result:
  • A laterally tightened, more clearly defined penile shaft
  • A youthful, compact appearance of the entire genital zone
  • No disturbing longitudinal folds beside the penis
  • Harmonious overall appearance — also in combination with other procedures
The 3D scrotal tightening — at a glance
  • Problem:
Longitudinal folds lateral to the penile shaft — excess skin beside the penis
  • Appearance:
Slack-looking, aging overall appearance of the genital area
  • Solution:
Gentle removal of this lateral excess skin
  • Developed by:
    Dr. med. Timo A. Spanholtz
  • Result:
3-dimensionally tightened, youthful genital zone — scrotum AND lateral folds
  • Combination:
Can be combined with the classic scrotal tightening in one session
Note: Especially valuable in combination with penis lengthening

Men who have planned or already undergone a penis lengthening particularly benefit from the 3D scrotal tightening: It brings the gained length result into a harmonious, coherent overall appearance — without disturbing lateral folds.

The 3D scrotal tightening can be performed in the same session as the classic scrotal tightening and/or the penis lengthening.

What scrotal tightening achieves — three results in one operation

  • Volume reduction: Excess scrotal skin is precisely removed — the volume of the scrotum is reduced: The skin closes more firmly and tightly around the existing volume.
  • Lifting of the testicles: The slackened scrotum is lifted and re-anchored — the testicles sit higher and the overall appearance seems firmer and younger
  • Penile web correction (optional): The skin between the penile shaft and scrotum is tightened — the penis comes into its full length

All three measures can be combined in a single operation. The result is permanent — the removed skin does not grow back.

The surgical procedure

Phase

What happens

1

Consultation

Detailed discussion with Dr. Spanholtz (via video or at the clinic) · Assessment of the extent of the excess skin · Clarification of whether a penile web is present · Discussion of combination options · Information about blood-thinning medications · Cost plan

2

Anesthesia

Short general anesthesia— depending on the extent of the procedure and individual preference · Discuss discontinuing blood-thinning medications beforehand

3

Skin removal & tightening

Excess scrotal skin is precisely removed through an incision in an inconspicuous area · simultaneous penile web correction on request · scrotum is lifted and re-anchored · Suture technique according to Dr. Spanholtz: scar lies exactly in the natural midline

4

Wound closure

Wound closure with self-dissolving sutures · Application of dressing · Home on an outpatient basis (with a driver) or one night in the clinic

5

Aftercare

1 week of absolute rest and consistent elevation of the scrotum — crucial for avoiding post-bleeding · Scar care with the provided products · Sex and sport: break approx. 4–6 weeks · Sutures dissolve on their own · Scars barely visible after a few weeks

Aftercare: Why one week of rest is indispensable
Due to its intensive blood supply, the scrotum is an area with an increased risk of post-bleeding. That is the reason why, after scrotal tightening, absolute rest and consistent elevation for one week are indispensable.
  • 1 week of absolute rest — please schedule a free week
  • Consistent elevation of the scrotum — decisively reduces the risk of post-bleeding
  • Scar care with the provided products — for optimal, inconspicuous healing
  • No sex and no sport for approx. 4–6 weeks
  • No blood-thinning medications (please discuss with Dr. Spanholtz in good time)

Light physical exertion is possible again after about 1–2 weeks.
The wounds heal in a short time — scars are barely visible after a few weeks.

Combination with other procedures in one session
Scrotal tightening can be excellently combined with other intimate surgical procedures — one anesthesia, one healing phase:
  • Penis lengthening (ligamentotomy) — especially recommended when a penile web is present at the same time
  • Penis thickening (autologous fat or Phallograft®) — for a harmonious overall result
  • Circumcision — a frequently chosen combination
  • P-Shot® or Penitox — erection improvement in the same session
  • Scrotal enlargement — for men who simultaneously want more scrotal volume

Which combination makes sense for you is discussed by Dr. Spanholtz during the consultation — individually, honestly, and without pressure.

Result: Firmer, younger, more proportional

Frequently asked questions about scrotal tightening

Does a visible scar remain after the operation?
The suture technique developed by Dr. Spanholtz places the scar exactly in the natural midline of the scrotum — an anatomical fold that naturally divides every scrotum. This scar is practically undetectable after complete healing. During the consultation you receive the necessary care products for an optimal healing result.
Does the operation affect the fertility or function of the testicles?
No — the testicles themselves remain untouched during the procedure. Only the excess scrotal skin is removed. Fertility, libido, and reproductive capacity are fully preserved. The temperature-regulating function of the scrotum is also maintained.
What is a penile web — and why is it relevant for penis lengthening?
The penile web is a skin connection between the lower penile shaft and the scrotum that disturbs the testicle-penis angle and makes the penis appear optically shorter. During a penis lengthening operation, the penile web should definitely be corrected at the same time — only then does the full length result come into effect optically.
How long is the downtime after the operation?
Please schedule 1 week of absolute rest and elevation — this is crucial for avoiding post-bleeding. Light physical activities are possible from week 2. Office work can usually be resumed thereafter. Sport and sexual intercourse should be paused for approx. 4–6 weeks.
Is there a non-surgical alternative?
Yes — Scrotox (botulinum toxin injection into the scrotum) is a minimally invasive alternative for men who do not want an operation. Scrotox temporarily smooths the scrotal skin and reduces sweating, but has no permanent effect and does not remove excess skin. For men with pronounced slackening or a penile web, the surgical solution is superior.
