AlphaFill® is a treatment for non-surgical penis girth enhancement using specially cross-linked hyaluronic acid. Hyaluronic acid is a substance naturally produced by the body, occurring naturally in the skin and connective tissue. In the AlphaFill® procedure, this high-quality substance is injected under the skin of the penile shaft — instantly giving it more volume and girth.

The hyaluronic acid stores water, distributes evenly under the skin, and thus creates a natural-looking, even thickening. It is gradually broken down by the body in a natural way — the result usually lasts 12 to 18 months and can be refreshed at any time.

How Much Girth Is Possible? Entry-Level and Intensive Procedures

You determine the desired extent of the thickening together with Dr. Spanholtz. The quantity of hyaluronic acid introduced depends on your wishes, the anatomy, and tolerability. In principle, there are two approaches:

The Course of the AlphaFill® Treatment

The treatment takes place on an outpatient basis at the private clinic K61. In total, you will need approx. 1 hour at the clinic:

Preparation & anaesthesia (approx. 20 minutes)
First, the penile root is anaesthetised with a local injection. Since the anaesthesia takes approx. 20 minutes to take effect, you can relax and have a coffee during this time. As soon as the anaesthesia is fully effective, the treatment is virtually painless.
The injection (approx. 20–30 minutes)
The quantity of hyaluronic acid previously discussed together is introduced into the penile shaft through small, precise injections — into the glans as well, if desired. This process is barely painful. Dr. Spanholtz distributes the material evenly to achieve a harmonious, natural result.
Bandage & aftercare
After the injection, a bandage protects the result for a few hours. It can be removed in the evening or the next morning. The result is immediately visible — the penis is noticeably fuller directly after the treatment.

Aftercare: What Should Be Considered?

So that the hyaluronic acid distributes optimally in the tissue and the result hardens evenly, several points are important in the first few days:

Physical rest on the day of treatment — no sport, no sexual intercourse
Avoid pressure on the penis for approx. 10 days — no tight underwear, preferably sleeping on your back
Sexual intercourse and sport: possible again from approx. 7–10 days after the treatment
Occasional slight swelling, redness, or small bruises are normal and subside within a few days

Freshly injected hyaluronic acid is still soft and can shift due to pressure.

Occasionally, sleeping on your side or wearing tight clothing can cause a slight maldistribution.

The Scientific Background

The question of penis girth and its significance is well researched scientifically. A widely cited study by King's College London — the university where Dr. Spanholtz completed part of his training — dealt with norm values for penis length and girth in over 15,000 men.

The study shows: there is enormous natural variation, and in an international comparison, German men are by no means poorly positioned. How Much Girth Is Achievable with AlphaFill®?

Typically, we achieve a girth increase of approx. +1.5 cm in one session. If a more intensive result is desired, we recommend the two-stage procedure with two injections two weeks apart — with this, girth increases of up to +3 cm are possible. The result is individual — depending on anatomy, the quantity introduced, and tissue properties.

Is the Treatment Painful?

No — thanks to local anaesthesia, the actual injection is barely painful. Only the anaesthetic injection at the penile root is briefly noticeable. Afterwards, you wait approx. 20 minutes until the anaesthesia is fully effective — and can relax and have a coffee during this time.

How Long Does the Result Last?

The results usually last 12 to 18 months. The hyaluronic acid is gradually broken down by the body in a natural way — after approx. 12 months, the thickening gradually diminishes. A refresh is possible at any time and without any problems. Anyone who wants more permanent results is better advised with a surgical method (autologous fat, Phallograft®).

Can the Result Be Undone?

Yes — this is one of the great advantages over surgical procedures. Hyaluronic acid can be completely dissolved with an enzyme called Hylase if the result does not meet expectations or a correction is desired. This step is simple, fast, and can also be carried out on an outpatient basis.

When Is Sexual Intercourse Possible Again?

Approx. 7 days after the treatment. During this time, pressure on the penis should be avoided so that the hyaluronic acid distributes evenly in the tissue and stabilises. After about a week, sexual intercourse and sport are usually possible again without any problems.
