The surgical procedure: Three steps to permanent girth enhancement

The operation is divided into three clearly defined steps — the entire procedure takes place under short anesthesia:

Fatty tissue extraction

The body's own fat is extracted through fine cannulas from the abdomen, thighs, flanks, or the pubic area (liposuction). In 99% of cases, sufficient suitable fatty tissue is found. Upon request, several extraction zones can be treated at the same time — as body contouring on request.

Fat preparation

The extracted fat undergoes a special processing: centrifugation, filtration, and enrichment. In doing so, fluid, blood, and damaged fat cells are removed — only the viable, high-quality fat cells remain. Upon request, the prepared fat is enriched with PRP (platelet-rich plasma from the body's own blood) — this improves the healing rate.

Transfer into the penis shaft

The prepared fat is injected precisely and layer by layer under the penis skin using finest cannulas — from the shaft to the root of the penis. Upon request, the glans can also be treated. No incisions are made on the penis — only tiny puncture points, which are no longer visible after complete healing.

The aftercare: The most important factor for the result

Autologous fat is a biological material — it must heal and integrate into the penis tissue. This process takes weeks and depends strongly on behavior after the operation. The aftercare rules are clear and non-negotiable:

Honestly explained: What about fat resorption?

No lipofilling procedure survives 100% — this is no different for penis girth enhancement than for other body areas. A part of the injected fat (typically 20–40%) is resorbed by the body during the healing phase. This is a natural and expected process.

How much girth increase is realistic?
This depends on the amount of fat introduced and the individual healing rate. Typically, a girth increase of +1.5 to +2.5 cm is aimed for — after taking resorption into account. Dr. Spanholtz plans the amount so that the desired result is achieved after complete healing.
I smoke — can I still have the operation done?
In principle yes, but with a clear statement: For smokers, the healing of the fatty tissue is massively worsened. Nicotine constricts the blood vessels and decisively impairs the blood supply to the transplant. Dr. Spanholtz recommends completely abstaining from nicotine in any form — cigarettes, e-cigarettes, patches, chewing tobacco — for at least 8 weeks before and after the procedure.
When is the final result visible?
The result is immediately visible directly after the operation — however, initially difficult to assess due to swelling. After the swelling subsides and complete healing (approx. 3–6 months), the permanent result becomes apparent. A possible correction or top-up session is possible after approx. 3–6 months.
