Step 1 in detail: The Penis Lengthening

The most common and effective method for permanent penis lengthening is the proper ligamentotomy of the central ligament area (surgical errors are often made here!) — the severing of the Ligamentum suspensorium penis, a supporting ligament that connects the penis shaft to the pubic bone.

The penis has an actual length that is greater than the externally visible part — one part lies inside the body. By severing this ligament, this hidden portion is released and the penis shaft is shifted further outward. This requires a high degree of surgical experience to secure a permanent result.

Step 2: Erection improvement as part of the concept

What distinguishes PHALLOMAX® from a pure lengthening or thickening procedure: erection improvement is an integral part of the overall plan. As part of the first procedure, Penitox (botulinum toxin into the erectile tissue) and/or P-Shot (PRP stem cell therapy) can be performed.

Step 3: Penis Thickening with Autologous Fat
About 10 weeks after the lengthening procedure, the thickening takes place. The procedure: the body's own fat from the abdomen or hip is extracted, gently processed, and injected in precise layers under the skin of the penis shaft. Optionally, the glans can be enlarged in the same session.
Advantages of autologous fat penis thickening
  • 100% of the body's own material — no foreign material, no risk of allergy
  • Permanent result — ingrown fat is retained (after complete healing)
  • Hardly any scarring — only the smallest incisions needed for fat extraction
  • Liposuction at the extraction area as a side effect — light body contouring included
  • Result: typically +1.5 to +2.5 cm penis girth
  • Note: A portion of the injected fat is absorbed by the body (approx. 50%). This is taken into account when planning the amount of fat to be introduced.

The aftercare: Phallosan Forte — crucial for the result

The penis lengthening is only the first step — consistent aftercare is equally important for the result. After the surgery, the severed ligament must be prevented from contracting again through external traction.

Dr. Spanholtz uses exclusively the Phallosan Forte — an extender device in the expander version that fixes the penis over the shaft and the glans. The device can be applied as early as 5–7 days after surgery and, due to its high wearing comfort, allows long daily application times.

For lighter activities, no longer downtimes are necessary. Sports and special activities such as sauna visits or swimming in cold water are individually coordinated with Dr. Spanholtz in the treatment plan.

Frequently Asked Questions about PHALLOMAX®

How long does the entire PHALLOMAX® treatment take?

The treatment extends over several weeks: the first surgical procedure (lengthening + erection improvement), followed by aftercare with Phallosan Forte, and after about 10 weeks the penis thickening. The active surgical phases are short — the recovery and aftercare time is the larger portion in terms of time.

How long of an inability to work should I plan for?
For office work, no long downtimes are necessary — many patients return to office work after a few days. For physical work, several weeks of rest should be planned. Sports are paused for an individually determined period. Dr. Spanholtz provides the exact recommendations after the procedure.
How dangerous is penis lengthening?
In experienced hands, the risk profile of the procedure is to be classified as manageable. Possible complications such as swelling, secondary bleeding, or scarring are generally treatable and temporary. Dr. Spanholtz will inform you in detail about all risks during the consultation. Important: poor results often arise from inexperienced surgeons — choose your surgeon carefully.
When is sex possible again after surgery?
Sexual activity is generally possible again after complete wound healing — this varies according to the individual healing process. Dr. Spanholtz will give you specific recommendations after the procedure.
Are there age limits for PHALLOMAX®?
The minimum age is 18 years. There is no upper age limit — Dr. Spanholtz has successfully performed penis enlargements even on patients in their seventies. What is decisive is a good general state of health and fitness for anesthesia.
